It’s Flu vaccination season!
Attached you’ll find a consent form for resident vaccination and the information on this year’s vaccine. Please read and choose if you want your loved one to receive the vaccine this year or not and return it to the facility by fax, mail, email, or in person. We offer the vaccine to all staff and residents. If you have any questions or concerns, please call the facility.
